LINKS IN DESCRIPTION! CentriKid is 5 days of unbelievable fun, friends, food, and crazy activities in an amazing safe environment at Jenness Park (near Sonora). Make plans to send your 3rd through 5th grader with us! WHEN: July 5-July 9 (Meet at PVBC at 9am on July 5th. Pick up students at PVBC mid to late afternoon July 9th.) WHERE: Jenness Park COST: $340 PACKING LIST BELOW PARENT MEETINGS (choose one) Sunday, June 5th @ 12:30pm Monday, June 6th @ 7pm Get your forms notarized! We'll have info and refreshments for you! STEPS TO GO TO CAMP: 1.Fill out online form at bit.ly/registercentrikid 2. Print and fill out the CentriKid form at bit.ly/centrikidform 3. Have each CentriKid Form notarized (IMPORTANT) 5. Print and fill out both pages of the PVBC waiver at bit.ly/pvbcform (Does NOT need to be notarized.) 6. Make a copy of student’s medical insurance card 7. Cost is $330 per student. If you need a partial or full scholarship, please let Pastor Michael know. Write check for full amount (or remaining balance after scholarship) to PVBC. 8. Mail or bring BOTH completed forms, copy of insurance card, and check to PVBC 875 S. Park Victoria Dr., Milpitas CA 95035 WHAT TO BRING: Sleeping bag / bed linens (whichever you prefer) Blanket and pillow Towel for showering 6 days worth of clothing (gym shorts, t-shirts, tennis shoes for during each day and casual attire for nightly activities and worship services each night) Swim suit (one piece) Closed toe, closed heel shoes (for outdoor activities / rec time MANDATORY) Bible, notepad, and pen Toiletries (shampoo, soap, toothbrush, toothpaste, deodorant, etc.) Sun block Lunch and snack money to and from camp (2 meals total) Money for snacks, camp store, and smoothie bar Money for missions offering WHAT NOT TO BRING: No clothing that has spaghetti straps or is excessively short or tight-fitting Alcohol, tobacco, illegal drugs, fireworks, water guns, or any kind of weapon Any clothing that advertises the above or refers to sexual situations Cell phones, iPods, MP3 players, and gaming devices may be brought for the ride to / from camp, but will be collected from you when you arrive at camp and will be returned to you for the ride home Any questions?
